At any age, a medical crisis can leave 1 too ill to express his or her healthcare wishes, making it difficult for family, friends and healthcare providers to know what best to do in difficult and possibly irreversible situations. A written Advance Health Care Directive (AHCD) allows a person to express his or her preferences in such situations and allows the individual to designate a family member or friend to make medical decisions if 1 is incapacitated and unable to do so. A POLST (Physician Orders for Life-Sustaining Treatment) is another document that allows a person to specify what he or she wants for end-of-life care.
