NGC 2000-P Jefferson nickel dual-date double-denomination, struck on a 1961 Lincoln cent. The coin shows the two dates clearly, as well as design detail from both the cent and the nickel strikes. It is very rare to have dual-date double-denominations for nickels on cents, but it is even more rare to have them with this many years apart. In fact, we only know of a total of 3 examples with years more than a decade apart, with this being one of those three, and also the farthest apart (39 years total!)
