This is a USB 2.0 card reader / writer from GE, model 97297. This reader supports the following formats through one of four different slots: MicroSD, SD, SDHC, Mini SD, MMC, MS, MS Pro, MS Duo, and M2. It has been tested on Windows 7, and it works fine. The USB connector has some rust on it (does not interfere with operation) that needs cleaned off somehow. See pictures for details.
